Job Summary and ResponsibilitiesAs our Geriatric Nurse Practitioner, you will help older adults, particularly those with Alzheimer's and Dementia, receive specialized, comprehensive primary care and off-site home visits so they can improve their health outcomes, manage complex geriatric conditions, and maintain a higher quality of life.
In this role, you will focus on Alzheimer's and Dementia care, assuming clinical responsibility for patient histories, physical examinations, accurate diagnoses, and care management. You will be expected to manage a project volume of 250 patients, conduct new consults and follow-up visits in clinic and through home visits, and collaborate within a multidisciplinary team to ensure holistic patient care.
To be successful in this role, you must be a licensed Nurse Practitioner with expertise in geriatrics and dementia care, demonstrate exceptional proficiency in clinical assessment and management, and possess strong communication and collaborative skills for diverse patient and team interactions.
- This position will be Alzheimer's and Dementia Care focus
- Patients are seen in the office with some off-site home visits in the surrounding area
- Typical work week (hours worked; hospital vs. clinic) - option to work 4 or 5 days per week
- Working in clinic setting, as well as, off site visits throughout the community to accommodate the patient's needs
- Typical patient visit load of 2 new consults a day and follow up visit

Job Requirements
Required
- Current Board Certification
- Active or eligible license in the state of Arizona
- Valid DEA certificate
Preferred
- 2-5 years experience in geriatrics with focus on cognitive diseases preferred
