As a Gastroenterology Nurse Practitioner with CHI St. Vincent Medical Group, you will work alongside a supportive team of physicians, nurses, and administrative staff, providing comprehensive care to our community.
In your practice, you will provide direct patient care to individuals with a wide range of gastrointestinal disorders, including but not limited to inflammatory bowel disease, GERD, liver disease, and pancreatic conditions. This is an exciting opportunity to contribute to seamless patient care coordination through effective collaboration with gastroenterologists, primary care providers, and other specialists.
To be successful in this role, you will be a skilled and compassionate Gastroenterology Nurse Practitioner committed to providing high quality comprehensive care within a community focused practice. In your role, you will:
Conduct physical examinations, order and interpret diagnostic tests, and develop individualized treatment plans in collaboration with supervising physicians.
Perform medication management, patient education, and counseling on lifestyle modifications to promote optimal digestive health.
Manage acute and chronic GI conditions, monitoring patient progress and adjusting treatment as needed.
Collaborate effectively with gastroenterologists, primary care providers, and other specialists to ensure seamless patient care coordination.
Educate and empower patients to understand their conditions and actively participate in their treatment plans.
Maintain accurate and detailed electronic health records (Epic).
