As a physician with Providence Medical Group at Home, youll join a skilled, interdisciplinary team dedicated to providing Primary Care to patients in a home-based, community setting. The team is seeking a board-certified/board-eligible Family Medicine or Internal Medicine Physician part-time at 0.8 FTE (32 hours per week). In this role, you will provide Primary Care with an emphasis on management of chronic conditions, disease prevention and symptom management to patients who are not best served in a Primary Care clinic. Part-time; 0.8 FTE at 4 days per week
Monday to Friday; daytime hours, variable days
Outpatient Primary Care in a home-based, community setting
Work within an interdisciplinary team including pharmacy, care management, nursing staff
Ideal candidates will have previous clinical experience working in Primary Care, home health, Geriatrics, and/or palliative medicine
